Let us confirm your leads actually arrive.
Every day we send one clearly labelled test through your website form. The message always contains a code like FV-TEST-3f9a1c2e and the words "Automated FormVitals test". To confirm it reached you, we need to see that test email, and only that email.

Option A: inbox filter
Set a rule in the inbox that receives your leads so it forwards only emails containing FV-TEST to us.
Why it's better: this proves the lead reached your real inbox, not just that it was sent, and we never see your real customers' messages.
Option B: BCC on the form
Add our address as a BCC in your form's notification settings.
Trade-off: quicker if your web person is already in the form settings. We'll also receive real leads, which we automatically ignore and delete, but some businesses prefer Option A for privacy.
Option A: forward only test emails
Gmail / Google Workspace
- In Gmail, open Settings (gear) → See all settings → Forwarding and POP/IMAP.
- Click Add a forwarding address, enter the monitoring address, and confirm. We'll approve the verification on our side within a few minutes. Then leave forwarding itself turned off; the filter in the next steps does the work.
- In the Gmail search bar, type
FV-TEST, click the filter icon, then Create filter. - Tick Forward it to, choose the monitoring address, and optionally tick Skip the Inbox and Mark as read so tests don't bother you.
- Click Create filter. Done.
If forwarding is missing
Google Workspace admins sometimes disable forwarding. If the "Forwarding" option is missing, ask your admin or use Option B.
Outlook / Microsoft 365
- In Outlook on the web, open Settings → Mail → Rules → Add new rule.
- Name it FormVitals test. Condition: Message body includes
FV-TEST. - Action: Forward to the monitoring address. Optionally add Mark as read and Move to a "Tests" folder.
- Save.
Microsoft 365 heads-up
Microsoft 365 blocks automatic forwarding to outside addresses by default for many organizations. If our first test doesn't show up, your IT admin needs to allow it, or use Option B.
Other email providers
Most providers (Yahoo, Zoho, GoDaddy, Fastmail, and others) support a rule of the form "if message contains FV-TEST, forward to…". Look for Filters or Rules in settings. If you get stuck, email us and we'll walk you through it.
Option B: add a BCC to the form
Find your form tool below. Add the monitoring address alongside your existing address, and don't remove yours.
WordPress: Contact Form 7
- Contact → Contact Forms, then open your form.
- Go to the Mail tab. In Additional headers, add a new line:
Bcc: monitor@formvitals.com - Save.
WordPress: WPForms
- Edit the form, then Settings → Notifications.
- In Send To Email Address, add a comma and the monitoring address.
- Save.
WordPress: Gravity Forms
- Edit the form, then Settings → Notifications, then edit Admin Notification.
- Put the monitoring address in the BCC field.
- Update the notification.
WordPress: Elementor Pro form
- Edit the page and click the form widget.
- Open Actions After Submit → Email and add the monitoring address in the Bcc field.
- Update the page.
WordPress: Ninja Forms
- Edit the form, then Emails & Actions, then open the admin email action.
- Under Advanced, add the monitoring address to BCC.
- Publish.
Wix, Squarespace, Webflow, and others
These builders change their settings screens often, and some only send form notifications to account members. In the form's settings, look for Notifications, Automations, or Email and add the monitoring address if it's allowed. If it isn't, use Option A; it works with every website builder because it happens in your inbox.
Forms with "I'm not a robot" checks
If your form uses reCAPTCHA, hCaptcha, or Cloudflare Turnstile, our tests will stop at that check, and we never try to get around it. Your web person can either allow our test runner through, or switch that form to a "score-based" (invisible) CAPTCHA mode. We'll send exact instructions for your setup when you sign up.
What happens next
- We send the first test within a few minutes of setup and email you once it's confirmed.
- From then on it's one test a day, plus the other checks (uptime, security certificate, domain, email setup, accessibility).
- If anything breaks, you get an alert explaining it in plain English. Once a month you get a short health report.
